LifeCare will evaluate and address the physical, functional and environmental barriers that impact mobility and develop an individualized program to restore function.
ADL Assessment will include recommendations for adaptive equipment, energy conservation, pain reduction, activity or task simplification and ways to improve safety in the home.
Managing chronic conditions involves learning specific health-management skills. Our Team will help transfer medical recommendations to practice and ensure consistent and correct carryover into the home.
LifeCare will offer practical solutions to the patient and family to help maximize safety and function within the home and reduce the risks for falls and injury.
Patient's at risk for falls due to natural aging or chronic illness's experiencing a decreased ability to transfer, stand, ambulate, balance, and perform ADLs.
Vestibular Therapy Therapy is a program of specialized exercise-based treatments designed to improve inner ear deficits that cause dizziness and vertigo.
This Program is designed to train patient's sensory awareness on making large movements through exercises that target functional deficits through an interdisciplinary program of PT/OT and SLP services.
Designed to be a step-down program from Phase Ior Phase II cardio-pulmonary rehabilitation that is provided in the patient's home. Goals are to improve cardio-pulmonary fitness, integrate diaphragmatic breathing and improved energy conservation in all activities.
Studies show that almost three quarters of repeat hospitalizations could potentially be avoided. Our program prepares patients and caregivers to more effectively manage changes in health associated with hospitalization and reduce risk and need for re-admission through early mobilization, health monitoring, and family education.
LifeCare utilizes objective clinical measures to establish baseline status and report on improvement. This research-proven approach progressively moves a patient from lower to higher levels of mobility and functional independence. Patient vitals are assessed at each visit with immediate reporting of risk values.
